When weekly therapy is not enough, the next step may feel unclear. You may hear terms like PHP and IOP and wonder what they mean, how they differ and which one fits your needs. PHP stands for Partial Hospitalization Program, also called our Day Treatment Program. IOP stands for Intensive Outpatient Program. Both offer structured mental health treatment while allowing clients to return home after programming. 

Choosing a higher level of care does not mean you have failed. It may mean your symptoms need more structure, support and clinical time than standard outpatient care can provide.

What is PHP?

A Partial Hospitalization Program, also called our Day Treatment Program, is a structured outpatient level of care. Clients attend treatment during the day and return home afterward.

Medicare describes Partial Hospitalization as a structured program that provides outpatient psychiatric services as an alternative to inpatient psychiatric care. It is more intensive than care from a doctor’s office or therapist’s office. 

What is IOP?

An Intensive Outpatient Program is also structured outpatient care, but it usually requires fewer treatment hours than PHP. IOP may be a step down from PHP or a step up from weekly therapy.

How PHP and IOP are similar

PHP and IOP both offer more support than weekly outpatient therapy. They may include individual sessions, group sessions, psychiatry, family support when appropriate and a personalized treatment plan.

Intensive Outpatient or Partial Hospitalization can include one-on-one appointments, group sessions and coping skills education. These programs coordinate care more intensively than standard visits.

Both levels may help if symptoms are affecting:

  • Work — You are struggling to focus or show up
  • School — Daily demands feel hard to manage
  • Relationships — Conflict, withdrawal or distress is increasing
  • Safety — Self-harm urges or suicidal thoughts need support
  • Daily routines — Sleep, meals and self-care feel harder

How PHP and IOP are different

The main difference between PHP and IOP is intensity. PHP is usually more time-intensive and may be a fit when symptoms need more daily structure. IOP usually offers fewer hours and more flexibility.

In general:

  • PHP — More structure, more treatment hours and more daily clinical support
  • IOP — Structured support with more room for work, school or home responsibilities
  • Residential Treatment — 24-hour support in a home-like setting
  • Weekly outpatient care — Less intensive support, often one session per week

When PHP may be the better fit

PHP may help when symptoms are making daily functioning difficult, but 24-hour care is not needed. It may also help after Residential Treatment or inpatient hospitalization, when someone needs structure before returning to a lower level of care.

PHP may be worth asking about if:

  • Your symptoms feel hard to manage most days
  • Weekly therapy does not feel like enough
  • You need regular psychiatry or group support
  • You are stepping down from a higher level of care
  • You can safely return home after treatment

When IOP may be the better fit

IOP may help when you need more than weekly therapy but still have enough stability to manage daily responsibilities. It can also help as a step-down level after PHP.

IOP may be worth asking about if:

  • You need support several days per week
  • You can keep some work, school or home routines
  • You want structured group support
  • You are practicing skills in daily life
  • You need help maintaining progress after PHP

When to ask about a higher level of care

It may be time to ask about PHP or IOP if symptoms are getting harder to manage alone. This may include depression, anxiety, trauma-related symptoms, personality disorder symptoms, self-harm behaviors or suicidal thoughts.

A mental health evaluation can help clarify whether PHP, IOP or another level of care may fit.
